I know and realize the advantages, risks and probable after effects of the medicine(s) that are requested by me as I was advised by qualified medical staff.

I confirm that the medicine(s) I request have been securely used by me under surveillance of my local doctor or the doctor has informed me that the use of the medicine(s) is suitable for me and not counter-indicated.

The medicine(s) I request are only for my individual medical use and will not be given or sold to other people.

Law allows me to use the credit card which I will use in case of approving and processing my request.

I recognize, admit and consent to the statements below:

I allow RX Corp and the prescribing doctor to access any of my medical information and all health details provided in the Medical History form.
